Welcome to the Consultation practice of Dr. Patrick Mulcahy. Please read through the following prior to making an appointment.
Terms of Service
I understand that I will need to have an established Primary Care practitioner prior to making an appointment with Dr. Patrick Mulcahy. I will provide that person's name and contact information at the time of each consultation and I understand that Dr. Mulcahy will not serve as that Primary Care practitioner.
I understand that Dr. Mulcahy is not a part of any insurance plans, including any commercial plans, Medicare, or Mainecare. Any referrals, prior authorizations, insurance claims, or tests that require scheduling or insurance approval will not be given. If needed, a Superbill will be provided for Commercial plan purposes. The only way to schedule and pay for an appointment is through the Consultations page on www.southernmaineintegrativehealth.com. New patient visits with Dr. Mulcahy are $300.00 for the first visit (60 minutes) and $165.00 for each follow-up visit (30 minutes). Those patients who've seen Dr. Mulcahy in his office practice within the past 3 years should select the follow-up visit option, unless you want an hour visit. All those who have not seen him in the office in the past 3 years, please select the New Patient option. You will have the choice of talking with Dr. Mulcahy via Google Meet or by phone. When making the appointment, you will indicate your preference of contact.
I have read the above information carefully and agree to the terms of this Consultation service. All new patients to Dr. Mulcahy will need to fill out the Adult Intake form or Child Intake form prior to the appointment.
